Active NYS Managed Medicaid health insurance
You must be currently enrolled in a New York State Managed Medicaid plan.
An unmet health-related social need for food and nutrition
You have a documented or identified need for food and nutrition support that isn't currently being met.
Consent to be screened and navigated by your Social Care Network
You agree to be screened by a trained navigator and connected to the right services through your SCN.
