Is this free?

Yes — our food and nutrition services are free for eligible NY Medicaid members. There is no out-of-pocket cost for the services themselves.

Who is eligible?

You must have active NYS Managed Medicaid, an unmet health-related social need for food and nutrition, consent to be screened by your Social Care Network, and live in New York State. All four must apply.

Do I need Medicaid?

Yes. This program is specifically for members of NYS Managed Medicaid plans. If you're unsure about your plan, call us and we can help you figure it out.

What types of services are offered?

Medically tailored meals, medically tailored groceries, cooking supplies, and nutrition counseling and education from trained navigators.

How do I get screened?

A trained Star EIP navigator will walk you through the AHC HRSN Screening Tool. You can do the screening by phone, video, text, or in person.

How quickly can services start?

After you reach out, we contact you within five days to begin arranging your care. Actual service start times depend on your needs and your local Social Care Network.

Can a provider or family member refer someone?

Yes. Providers, partners, and family members can make referrals on behalf of someone else using our referral form or by calling us directly.
