A trained navigator contacts the patient you referred within five days of receiving your referral.
We screen with AHC HRSN
We walk them through the AHC HRSN Screening Tool by phone, video, text, or in person — their choice.
We navigate and report back
If eligible, we navigate them to services through their Social Care Network and keep you in the loop.
