Four things determine if you may qualify for free food and nutrition support through your New York Medicaid benefit. A navigator walks you through it.

Active NYS Managed Medicaid health insurance

You must be currently enrolled in a New York State Managed Medicaid plan.

An unmet food or nutrition need tied to your health

For example: managing diabetes, heart disease, or another chronic condition; a high-risk pregnancy; recovery after a hospital stay; or food insecurity during a major life transition. A navigator confirms the need through a quick screening.

Consent to be screened and navigated by your Social Care Network

You agree to be screened by a trained navigator and connected to the right services through your SCN.

Residence in New York State

You live in New York State.

A few more details.

Managed Medicaid

Most New Yorkers on Medicaid are enrolled in a Managed Medicaid plan — you may hear it called your “health plan” or your “managed care organization.” If you’re not sure, call us at (718) 787-1023 and we’ll help you find out.

How long services last

Food and nutrition services typically run for up to 6 months. If you need more support after that, your navigator helps you find it.

Cost

There is no cost to you. Services are paid for through New York’s 1115 Medicaid Waiver, which runs through March 2027.
