This site is for Star EIP LLC Food & Nutrition Services, a program delivered under the NYS Medicaid 1115 Waiver through the Social Care Network model. This policy explains what information we collect through this website and how we use it.
Information we collect
When you submit the referral or intake form, we collect the information you enter — your name, phone number, email, preferred contact method, county, Medicaid status, health plan (optional), whether you are referring yourself or someone else, a brief need description (optional), and your consent to be contacted.
How we use it
We use the information solely to contact you about Star EIP Food & Nutrition Services, screen you for eligibility, and — if you qualify — navigate you to appropriate food and nutrition services through your Social Care Network.
Who sees it
Submissions are received by Star EIP staff. We share information with your Social Care Network only as needed to deliver the services you’ve asked for, and only with your consent. We do not sell your information, ever.
How long we keep it
We retain form submissions for as long as necessary to provide services and meet program requirements, and then delete them in line with our retention practices.
Your choices
You can ask us at any time to stop contacting you, or to delete your information, by emailing hello@stareip.com or calling (718) 787-1023.
